Going home can be the moment everyone has been waiting for. It can also be the moment when five different plans collide: paperwork, medication instructions, equipment, transportation, and the person holding the house keys.
A calm discharge handoff brings those plans together. Use this guide for the transportation part of a Seattle-area hospital departure, while the clinical team determines whether wheelchair transportation is suitable and what care is needed during and after the trip.
Confirm the appropriate type of transportation
Ask the discharging team what travel position and support the rider requires. Do not assume that being discharged automatically means a standard wheelchair van is appropriate. If monitoring, treatment, a stretcher, or other clinical support is needed, the team should help arrange a service equipped for that need.
A routine ride request is not an emergency response. New or worsening symptoms belong with the care team before departure; use 911 for an emergency. The transport plan should follow the clinical decision, rather than determine it.
Define “ready for pickup”
A predicted discharge time is useful for planning, but it may not be the time the rider is ready to leave. Identify who will tell the transportation provider when the necessary departure steps are complete.
Ask the facility and provider to agree on a pickup location and communication method. At a large campus, include the building, entrance, and relevant discharge desk. Avoid sending a driver to a general hospital address with no contact who can resolve a question.
- Who confirms the rider is ready to travel?
- Where will the rider wait with the support they need?
- Who escorts the rider to the agreed handoff point?
- Who contacts the provider if departure is delayed?
Keep the care plan and the ride plan connected
Medicare’s discharge planning checklist prompts patients and caregivers to discuss medicines, equipment, daily activities, and follow-up care before leaving a facility. Bring transportation questions into those discussions so instructions are understood before the van arrives.
For the ride itself, make a belongings list: mobility device, personal items, discharge documents, and equipment the care team says must accompany the rider. Confirm who is responsible for each item. Ask the clinical team how any prescribed equipment is to be managed; a transportation driver should not be assumed to provide clinical care.
Check the home entrance before leaving
Imagine the arrival in ordinary detail. Who opens the door? Which entrance is accessible? Is the elevator working? Does a family member need to meet the rider? If the planned route includes stairs or a transfer, resolve that with the care team and provider in advance.
Explain exactly where transportation assistance ends and the receiving person’s support begins. The door-to-door assistance guide offers wording that makes this boundary easier to confirm.
Use one handoff note
Suggested note: pickup building and entrance; facility contact; readiness update method; chair and equipment details; approved assistance arrangement; destination entrance; receiving person and phone number.
Share this note only with people coordinating the trip. Keep unnecessary medical history out of general messages. A practical handoff needs enough information to support the journey, with sensitive care instructions handled through the appropriate care channels.
Plan beyond the first arrival
Before everyone disperses, identify who will arrange follow-up appointment rides. Save the working entrance instructions and update anything learned during the first trip. Our booking checklist can become a reusable starting point.
The destination is more than an address. A successful handoff means the rider arrives at the agreed place with the agreed support ready.
